As part of our commitment to Health and Safety, we have put together this document for you to acknowledge and sign on the day.

I understand that there are risks involved in the activities I will be undertaking.  I also understand that an unpredictable or uncontrollable event may occur that could possibly cause me harm.

 

I acknowledge that Oak and Thistle Ltd will take reasonably practicable steps to keep me safe while I am involved I these activities.  However, I accept full responsibility for my own actions or in-actions.

 

To the fullest extent permitted by law, I hereby waive any rights I may have to take legal action against Oak and Thistle Ltd, or other participating organisations , sponsors (including directors, officers and employees of all the organisations listed above) and all other event participants with respect to any personal injury or property damage whether arising from negligence or otherwise, as a result of participation in an event or workshop run by Oak and Thistle Ltd.

I understand that failure to comply with any of these conditions may result in the Manager of the Event requesting me to leave.

I agree to comply with the following terms that refer to my participation in all workshops and activities:

1. I have notified the Manager of the Event of any relevant medical conditions and pre-existing injuries, and I consent to the Manager of the event rendering or authorising such medical treatment as necessary and accept responsibility for all associated expenses.

2. I am an event participant and not an employee of Oak and Thistle Ltd.

3. I will not smoke, consume or store alcohol or illicit drugs while working on an event site.

4. I will respect the rights, feelings and property of all others associated with the event.

5. I will cooperate with the Manager of the Event to ensure a safe, happy and hygienic team environment.
